Parents and Athlete Team Waiver For Origin Sports Academy

Acknowledgment and Assumption of Risk:

I, the undersigned participant and, if applicable, parent/guardian of the participant, hereby acknowledge that participation in basketball activities with Origin Sports Academy (hereinafter referred to as "Origin") involves inherent risks and hazards, including but not limited to physical injury, illness, or property damage. These risks and hazards may result from the actions or inactions of other participants, coaches, officials, or the conditions of the facilities.

I understand that participation in basketball activities with Origin is voluntary, and I choose to participate despite the risks. I hereby assume all risks associated with my or my child's participation in these activities.

Waiver and Release:

In consideration of being allowed to participate in basketball activities with Origin, I, for myself and, if applicable, my child, hereby release, discharge, and hold harmless Origin Sports Academy, its officers, directors, coaches, volunteers, agents, and all other persons or entities associated with Origin, from any and all claims, liabilities, damages, or injuries (including death) arising out of or in connection with participation in basketball activities, whether caused by the negligence of Origin or otherwise.

I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Origin, including but not limited to its coaches and volunteers.

Medical Authorization:

I, the undersigned participant or parent/guardian, authorize Origin to secure and consent to any necessary medical treatment in the event of illness, injury, or medical emergency. I understand that Origin will make reasonable efforts to contact me before obtaining medical treatment but may do so if they believe it is necessary to protect my or my child's health and safety.

Photography and Publicity Release:

I grant Origin the right to take photographs, videos, and other media during basketball activities and to use these images for promotional purposes, including but not limited to social media, websites, and print materials.

Code of Conduct:

I understand and agree to abide by Origin’s code of conduct and rules, as outlined in Origin's policies and guidelines.

Assumption of Responsibility:

I understand that it is my responsibility to be aware of and comply with all Origin rules, policies, and guidelines and to communicate these to my child if applicable.

Acknowledgment of Understanding:

I have read this waiver, release, and medical authorization, and I fully understand its terms. I understand that I am giving up substantial rights, including my right to sue, and I sign it freely and voluntarily without any inducement.
